As easy as group is for underwriting, it is just that complicated to get.  In order to get a quote the first step you need is to have what is called a census.  Although the census does not need to have anyone’s particular name it does require that it lists the number of personnel with what dependence will be included.  Instead of an actual name you at use employee number one or employee number two, etc..  You would list their ages, and zip codes.  You would also lists the additional dependence that are going to be covered in the health plan with their ages and relationship to the primary ensured.

Most insurers would require some kind of federal identification as an DE 4 articles of incorporation, state disability information or something like that to prove that you are an actual group.
